This is my original version of this system. The "official" version lives at http://libweb.uoregon.edu/iris/kitc, which requires a password to access. The official version is similar, but my Perl script was ported to PHP by student techs (and some of the functionality was lost).

I finished working on this in June of 2002; as far as I know, it is still being used.

Go ahead and create a record if you want. Check kitc_requests.csv and/or sitc_requests.csv to see the new record in the output file.
